Developing a Craniofacial Bone Regeneration Method, Hendra Sanjaya Earns a Doctor of Medicine Degree

I Gusti Putu Hendra Sanjaya, Sp.B, Sp.BP-RE (K) with the dissertation title " Administration of Recombinant Human Bone Morphogenetic Protein-2 (rhBMP-2) and Chitosan Scaffold Increases Osteocalcin Expression, Osteoblast Count, Woven Bone Formation and Expression Platelet Derived Growth Factor (PDGF) in Wistar Rats' Calvarial Bone Defects."

 

Craniofacial bones with extensive defects due to infectious diseases, congenital abnormalities, cancer and also due to trauma require complex reconstruction and still have limitations in handling and often have the potential to cause morbidity in patients. Recombinant human Bone Morphogenetic Protein-2 (rhBMP-2) engineering technology has been developed for clinical use, but this role needs to be optimized by combining it with chitosan scaffold as a buffer medium. The aim of this study was to prove that the combination of rhBMP-2 and chitosan scaffold accelerates craniofacial bone regeneration in defects of the calvarial bone by increasing osteocalcin expression, osteoblast count, woven bone formation and PDGF expression.

 

This research was carried out with the concept that recombinant human Bone Morphogenetic Protein-2 is osteoinductive and chitosan has osteoconductive and osteoinductive properties as a scaffold. The characteristics of each material are expected to be able to synergize with each other in accelerating growth and bone formation which will help healing, especially in craniofacial bone defects.

 

The results obtained from the study with a post-test only control group design with 24 Wistar rats were that administration of chitosan, rhBMP-2, and a combination of rhBMP-2 and chitosan showed higher osteocalcin expression, osteoblast number, woven bone area and PDGF expression. significantly compared to the control group (p< 0.05). A significantly higher increase (p< 0.05) was found in the rhBMP-2 and chitosan combination group compared to the group using chitosan alone or the group using rhBMP-2 alone.

 

The development of a new science (novelty) from this research is the use of a combination of recombinant human Bone Morphoginetic Protein-2 (rhBMP-2) and chitosan scaffold in the calvarial bone defects of Wistar rats to be effective in the field of craniofacial bone regeneration, by increasing the expression of osteocalcin, the expression of platelet-derived growth factor (PDGF), the number of osteoblasts, and the formation of woven bone. The conclusion of this study was that the administration of a combination of rhBMP-2 and chitosan scaffold to the calvarial bone defects of Wistar rats accelerated the regeneration of the calvarial bone defects.
