Doctor of Medical Sciences Develops Family Empowerment Model in Management of Diabetes Patients

Located in the third floor Postgraduate courtroom, the Doctoral Promotion exam was held with the promovenda candidate Ns. Ni Wayan Trisnadewi, S.Kep., M.Kes with the dissertation title "Development of a Family Empowerment Model in Diabetes Management in Type 2 Diabetes Mellitus Patients."

The global prevalence of diabetes mellitus in 2019 was 9.3% which occurred at the age of 20-79 years, Indonesia is the sixth country in the world with the number of DM sufferers reaching 10, million and the prevalence of DM in Bali Province in 2019 was 60,423 cases. Until now, diabetes management is still not optimally implemented by patients considering the high incidence of diabetes complications. Family empowerment is an important aspect in supporting the successful implementation of diabetes management. This study aims to determine the factors related to diabetes management, the inhibiting and supporting factors of diabetes management, to develop a family-based education model in diabetes management through expert testing.

 

This study uses a Mixed Methods design with an Explanatory Sequential Design approach. This research consists of 3 stages: cross-sectional research, phenomenological study, model development and expert test (Delphi Study) on the model. Respondents in this study were type 2 DM patients. The study was conducted from September 2021-April 2022 in Gianyar Regency.

 

The first phase of the study with a sample of 207 people, got the results that three variables were statistically proven to be related to diabetes management in type 2 DM patients, namely good patient knowledge with an Adjusted OR value of 2.635 (95%CI 1.407-4.932), good family unit empowerment with Adjusted OR value of 3.636 (95%CI 1.943-6.803) and patient's perception of the absence of risk factors for DM with Adjusted OR value of 3.268 (95%CI 1.705-6.263).

 

The second phase of the study successfully interviewed 15 DM patients and got the results that the family supported instrumentally and informationally but did not provide assessment and emotional support as well as the empowerment of the family unit. Management carried out by participants in the form of eating patterns, consuming drugs and doing sports. Patients have hopes for their families to be involved in diet and treatment, play a role in helping treatment and expect families to be emotionally and spiritually involved in DM management.

 

Phase three research resulted in a family-based diabetes management module. A good family-based diabetes management module covers aspects: Self-Instructional, Self-Contained, Stand Alone, Adaptive and User Friendly aspects. This module is expected to improve diabetes management for patients and their families so that diabetes complications can be prevented and the patient's quality of life is improved.
